- Completed Application Form
- Application Fee
- Copy of Birth Certificate
- Copy of Baptismal Certificate
- Prior School Transcripts (last 2 years of report cards if applying for 1st-8th grade)
- Completed Transportation (Bus) Form
- Completed Health History – (must be submitted before school starts)
- Visit with School Administration

Re-registration for current families opens in December. Once our current families are registered we will know exactly how many seats are available in each class for the new incoming students (families). New students are accepted on a rolling basis.
