Please read the Sudden Cardiac Death in Young Athletes Pamphlet and print out the forms. The New Jersey Department of Education states that the forms must be filled out by parent and physician before a student can participate in sports at Our Lady of Hope School. First two pages are to be filled out by student and parent. The last three by the physician.
Forms need to be completed and returned (only for students in grades 6 through 8) to school office to the attention of the Principal.
